Among our acute and continuing care services, our team cares for 45,000 patients per year in the 24/7/365 emergency department, provides orthopedics and ophthalmology, operates a dialysis unit, and is renowned for its mental health programs and restorative seniors care. Department/Unit/Program Description

Kawartha Lakes Ontario Health Team (KL-OHT) is a fully designated Ontario Health Team for the City of Kawartha Lakes and surrounding communities. Our mission is to coordinate and connect care to create a seamless experience for patients and families. By putting the patient at the centre of our system, we aim to reduce confusion, support more efficient navigation between care settings, and help keep people healthier, for longer.

Position Summary

As part of the KL-OHT, you will have a unique opportunity to work alongside a dynamic team of highly-committed patient/client partners, care providers and leaders from across the continuum of care, and will work to pioneer a new approach to health care and related services. This position will play a key role in the transformation of health care access and delivery in the KL-OHT region. The ICPL will work closely with the KL-OHT Executive Director and multi-stakeholder working groups to develop innovative and co-designed models of integrated care and provide ongoing project management support in their implementation. This position will also work with the KL-OHT team (decision support and quality improvement specialists) to develop, identify, monitor, and analyze ongoing performance and population health metrics related to the various projects of the KL-OHT. While the specific projects in this portfolio will be varied based emerging KL-OHT priorities and team complement, they may include: 24/7 navigation supports; enhancing transitions in clinical pathways; digital health solution implementation; implementation of new models of home care delivery and coordination, etc.

This position is funded by and will report to the Executive Director of the KL-OHT. As a member of the KL-OHT, Ross Memorial Hospital (RMH) is supporting the recruitment for this role. Responsibilities:
• Provide support to KL-OHT working groups to identify and implement projects and workplan deliverables.
• Initiate and plan assigned strategic projects including the development of scope, goals, and objectives.
• Develop project charters, work plans, project schedules, cost estimates, and budgets as required.
• Engage key stakeholders to ensure project activities are executed based on plans and standards, within the specific scope, budget and schedule.
• Complete cross jurisdictional scans for emerging and promising practices related to the development of systems of integrated care.
• Support the implementation of various priority projects as required; for example, providing administrative and change management support as appropriate based on the project needs.

Essential Qualifications

• In depth knowledge of Ontario’s OHT model and local OHT structure/plan
• Experience influencing systemic change and quality improvement in healthcare environment.
• Proven track record of being able to deliver on project/program targets.
• Actively engages in the identification and use of health-related data to support planning.
• Ability to influence change in a dynamic established environment.
• Project management experience in the healthcare setting a strong asset.
• Works cross-collaboratively with groups across the KL-OHT to align approaches with other areas of work.
• Demonstrated good attendance and performance records with the ability to maintain these same standards.
• Excellent interpersonal skills, with a track level of engaging health system leaders.
• Self-starter, with outstanding customer service.
• Effective communicator, able to communicate effectively and concisely, both orally and in writing, including effective presentation skills.
• Experience in health care with a minimum of five years of experience.
• Excellent understanding of Ontario’s healthcare system and healthcare administration.
• Excellent problem-solving skills, negotiation, issues management, organizational, project management, decision making and analytical skills.
• Clinical or health care administrative background an asset, especially in a variety of care environments (Acute, Primary, Community).
• Must be able to fulfill the physical requirements of the job, as per the current Physical Demands Analysis
